Home Care Packages (HCP) are a government initiative designed to support older Australians in accessing affordable care and services within their own home. HCP provides a coordinated approach to delivering assistance — from help with daily tasks to more complex or intensive care needs.

Eligibility for a Home Care Package is determined through a face-to-face assessment, which can be arranged through My Aged Care. If deemed eligible, you will be asked to choose a service provider. Your chosen provider will then coordinate services based on your individual needs and approved funding level.
