Chronic Disease Management (CDM) Plans enable GPs to plan and coordinate care for patients living with chronic or terminal medical conditions. While there is no fixed list of eligible conditions, CDM plans are intended for individuals who require a structured, multidisciplinary approach to managing their health.

Patients who are deemed eligible can access up to five allied health appointments per calendar year under the Medicare rebate system to assist in managing their condition. A referral from your treating GP is required to access these services.
