Chronic disease management (CDM) plans enable GPs to plan and coordinate the health care of patients with chronic or terminal medical conditions. There is no set list of conditions eligible for a CDM however plans are designed for patients requiring a multi-disciplinary structured approach. 

Patients deemed eligible are entitled to five appointments per calendar year through the Medicare rebate system with relevant allied health providers to help manage their chronic condition. Eligible patients require a referral by their treating GP.
